Application Packet
Request
This service is for
individuals who wish to receive an application packet
for their own use in applying for licensure or
certification in the state of Nevada. Application
packet requests from individuals must specify the type
of application required – either for Residential
Care/Assisted Living or Nursing Home Administrator.
Each packet contains Instructions, Requirements,
Information, Application Form and Documents, and
Instructions regarding the testing procedure for the NAB
National Examination that is required to become licensed
in the State of Nevada. All requests must be
accompanied by cashier’s check, money order or business
check only (we do not accept personal checks or
credit cards) in the amount of $25.00 and mailed to:
